For your first appointment, please bring all documentation for:
– Proof of Income & Address
– Photo ID (please bring to all of your appointments)
– Birth certificate
– Social Security
Please arrive early to complete all necessary paperwork to determine your eligibility for public insurance programs.
Note: If you are more than 15 minutes late for your appointment, please be patient. We may have to reschedule your appointment, if necessary.
Walk-In Appointments: We offer walk-in appointments at all of our clinics on a first come, first serve basis. We also offer convenient care hours (after 5pm) at our Mowry II Location.
If you are very ill, we may recommend that you go to the Emergency Room.
Translation: Many of our staff members who work with patients can speak other languages besides English-
including Spanish, Mandarin, Cantonese, Punjabi, Farsi and others. If you need a translator, we will provide you with one.
Children: The safety of your children is very important to us, please keep them with you at all times.
Youth and children under 18 cannot be seen at our clinics without their parent or legal guardian present, unless at the Teen Clinic for Family Planning Services.
Check-In: Please check-in with the front desk when you arrive.
– Liberty Clinic: you will receive a ticket with a number on it; please hold on to it and watch the monitor for your number.
– Other sites: front desk will check you in and your name will be called to see the medical provider.
Restrooms: All sites have restrooms available for your use. Please notify the front desk if the restrooms are not clean.
Isolation: If you or your child has a rash or a cough, please tell the front desk immediately.
You may be asked to wait in a different room from the waiting room or to wear a mask.
This is for both your protection, and/or your child’s, as well as for the protection of others.
Pet Policy: Please leave your pets at home when you come to any of our clinic sites.
Services animals needed by people with disabilities are acceptable.
