From the moment you arrive at our community clinic you become part of a long tradition of distinguished health care. We aim to provide the highest quality health care and service to all of our patients. We want your experience at TCHC to be as safe, pleasant and as comfortable as possible. 39% of our patients are best served in a language other than English. We have staff members who are fluent in 21 different languages and dialects. If you need someone to translate for you, we will provide a translator.
To become a patient at Tri-City Health Center, please contact our appointment line at (510) 770- 8040. When you call, our Member Services department will work with you to identify your eligibility for free or low cost medical services and/or insurance programs. We will make an appointment with a doctor, a nurse practitioner or a physician’s assistant.
For your first appointment, we will need you to bring some information with you, including:
- Proof of your income- if you have dependents, you must bring proof of their income as well.
Note: If you do not have any income, you will need to bring a Letter of Support from whoever you are living with.
The Letter of Support should say what kind of support they are giving to you (a place to stay, food, etc.) and have their name and phone number on it.
- Proof of your address- if applicable
- Photo ID
- Birth Certificate- if asked to
- Social Security Card- if applicable
TCHC Sliding Scale Program:
Tri-City Health Center serves all patients regardless of ability to pay. Ability to pay is determined by household income and family size and is applied to all patients, including those with insurance. For more information, ask at the front desk. Thank you.
