Homeless Programs

Tri-City Health Center provides homeless citizens living in our community with access to critically needed health care.  We have community case workers, who are able to assist homeless individuals and families living in our community by keeping them informed about the medical and social services we can provide for their health care needs.  Individuals needing medical attention have access to a physician, nurse practitioner, or a physician assistant at our three main clinics or six satellite sites.  In addition to providing medical services, TCHC also provides:

  • Access to housing assistance and support services
  • Case management
  • Financial benefits counseling
  • Lab work
  • Prescription medication
  • Radiology

HOPE Project Mobile Health Clinic
In addition, Homeless individuals can receive services through the HOPE (Homeless Outreach for People Empowerment) Project. The HOPE Project Mobile Health Clinic, a partnership with Abode Services, provides health and social services to people in southern and eastern Alameda County experiencing homelessness.
