Tri-City Health Center offers someone for our patients to talk with for support in achieving their behavioral health goals. We work with our patients to provide consultation and assessments in individual and group sessions.
Our Behavioral Health providers work as a team with our patients and their primary provider as we focus on our patients’ well-being as a whole person within their family, culture and community. Good health care involves paying attention not only to physical health, but also to habits, behaviors, thoughts and emotions. Together we look at topics such as lifestyle and behavior issues, stress reduction and mindfulness.
For our patients’ general well-being, as well as to address specific health concerns, our Behavioral Health team provides services in the following areas:
- Chronic Illness, such as asthma, diabetes or high blood pressure
- Chronic Pain
- Coping with HIV positive status
- Depression / Anxiety
- Grief, Loss or Bereavement
- Prenatal Care
- Smoking Cessation
- Weight Management
TCHC’s primary care providers will work with each patient to decide whether Behavioral Health services should be incorporated into the patient’s care plan.
