History

Tri-City Health Center (TCHC) was originally founded in 1970 as a family clinic for low-income minority women living in southern Alameda County, including the communities of Fremont, Union City and Hayward. TCHC became a Federally Qualified Health Center (FQHC) in 2003 and now offers primary medical, dental and behavioral health services for the entire family through nine sites, including 5 Fremont-based primary care sites, a dental site and a mobile clinic.

TCHC is the only FQHC in the City of Fremont offering a health care home which emphasizes disease prevention and chronic disease management. TCHC’s comprehensive menu of services include: primary care, pediatrics, prenatal, women’s health, HIV/Hep C, chronic disease management, nutrition counseling, family planning, behavioral health, dental, vision, care coordination, transgender health, and health insurance counseling and enrollment. Recognizing the rich diversity in our community, Tri-City proudly offers comprehensive services and specialty care programs delivered by a capable and caring staff that speaks 21 different languages.
